1. In view of destruction to lives and properties brought by super typhoons Iliang and Loleng which hit the country recently, all concerned are hereby requested to report the extend of damages of their offices, school building and other structures, equipment and instructional materials.
2. The report should consist of two (2) parts. The first part will cover only properties insured with the Government Service Insurance System, indicating the types of buildings, year and cost of construction and model, year and cost of acquisition in the case of equipment. It shall be accomplished in quadruplicate using the GSIS Form for Property Replacement Fund. The second part will be a summary of all damaged buildings, equipment and instructional materials including school desks and textbooks, irrespective of whether these are insured or not.
3. Inclosed are the forms to be used for the second part of the report. The deadline for the submission of the report shall be on or before December 18, 1998.
4. Strict compliance with this Order is hereby directed.
