1. It has been observed that there are students submitting tampered or fake NCEE results. In some instances the cases are discovered only when the students are about to graduate or when they are applying for special order. This creates more problems on the part of the administrations, parents and students. To prevent such incident, effective this School Year 1990-1991, all forms 137 shall bear the NCEE results of the student and shall be mailed direct to the registrar of the school where the student is enrolled.
2. The Form 137 shall be placed in an official envelope with white tape over the flap. The principal has to sign three times across the sealed flap of the envelope before it is mailed.
3. Widest dissemination of this Order is enjoined for the strict compliance of all concerned.
