Record of module use and monitoring of learner’s progress

In this form, the learner records each learning module he or she completed, the learning strand to which the learning module belongs, the date of accomplishment, the results of the pre-test and post-test, and other related activities accomplished. The learner is asked to reflect on his or her performance, identify personal strengths and weaknesses, and suggest ways to overcome difficulties that may have been encountered. The learning facilitator will also evaluate the learner’s performance and provide feedback on the learner’s level of mastery of the module contents. This form can be accomplished in Filipino or in English.
